Terms and Conditions

Member card is only valid for person mentioned on the card (ID and member card
is needed upon registration and payment). |

Member benefit will expire at the date of validation on the membership card. |

Full benefit of being a member will start 30 days after registration. During the first
30 days of register, member's benefit is only 5% from total bill (probation period). |

Validity of 1 year member is 30 days probation + 12 months full benefit. |

Validity of 6 months member is 30 days probation + 6 months full benefit. |

Benefits of membership DOES NOT apply for insurance holders. |

Single member = 1 person, Couple member = 2 people, family member = 3 - 4
people (Father, mother & 1 or 2 children). |

Free basic medical check-up for 0 usage of the yearly membership. |

Benefit of member can only be used at Kasih Ibu Hospitlal Denpasar. |

All personal information of member will be kept secure and confidential. |
